Ulcer, Gastrointestinal

Description
A small erosion in the gastrointestinal tract. The most common type are duodenal ulcers (the first part of small intestine), and gastric ulcers (stomach). An infection with the bacteria Helicobacter pylori can cause ulcers. A biopsy may be done to rule out a cancer causing the ulcer. Other causes include: NSAIDs, alcohol consumption, and smoking. Duodenal ulcers are almost always non-cancerous. Stomach ulcers may become malignant. An ulcer is also called peptic ulcer.

Symptoms
Upper abdominal pain, vomiting blood, vomiting coffee ground material, bloody stools, black stools (melena), weak pulse, pale color, lightheadedness, fainting.

Tests
A history and physical exam will be performed. Tests to establish the diagnosis, the cause of the ulcer, and the extent of the disease may be done including: blood test for antibodies against Helicobacter, urea breath test, tissue culture for helicobacter, biopsy, and EGD (Esophagogastroduodenoscopy).

Treatment
Specific therapy depends on the severity of the disease but may include: histamine 2 blockers (cimetidine/Tagamet, ranitidine/Zantac), proton pump inhibitors (omperazole/Prilosec, pantoprazole/Protonix), endoscopy, intravenous fluids, endoscopy, and/or blood transfusion. If the Helicobacter pylori bacteria is identified medications to kill the bacteria will be given.
