Subungual Hematoma (Blood Under Nail)

Description
A subungual hematoma is a collection of blood underneath the nail. It occurs after trauma to the nail and is sometimes associated with a fracture of the finger. The symptoms of pain worsen as the blood accumulates and causes increasing pressure on the finger.

Symptoms
Pain in the finger, blue-purple discoloration underneath the nail, throbbing pain.

Tests
A history and physical exam will be performed. A X-ray may be done to identify an associated fracture.
- X-Ray
Treatment
Drainage (trephanation) of the hematoma by drilling or burring a hole in the nail is the standard treatment. Pain medications such as acetaminophen (Tylenol), and/or nonsteroidal anti-inflammatory medications/NSAIDs (ibuprofen/Motrin or Advil, naproxen/Naprosyn) are prescribed. A finger guard may be applied to prevent further injury during the healing process.
