Child Abuse

Description
Physical child abuse refers not only to multiple fractures at different ages, but also includes numerous other signs of injury. Physical abuse tends to occur at moments of greatest stress, when someone strikes out in anger at the child. Many of those who commit physical abuse have been abused themselves as children.

Symptoms
Fussy, poor feeding, vomiting, lethargy, seizure, burns, unexplained unconscious infant, bruising, burns, bite marks, bulging fontanelle.

Tests
A history and physical exam will be performed. The diagnosis relies on the discovery of injuries not compatible with the history given. Finding multiple fractures of different ages helps confirm the diagnosis. Head and abdominal CT scans may be performed to rule out other serious injuries.

Treatment
Depends on the injury. The most important feature is removing the child from the dangerous environment, and involving the child protective services agency.
